In recent years, tumescent liposuction has become more accepted because it is less invasive than traditional liposuction, and involves the use of only local anesthesia. Your surgeon will inject a solution (including lidocaine, salt solution, and epinephrine) into the deeper fatty tissue layers through a small incision. The volume of fluid will be roughly three times the amount of fat to be removed. 

This solution will isolate the fat cells by causing them to swell. At the same time, the blood vessels in the area will shrink because of lidocaine. Your surgeon can precisely remove the isolated fat cells, while you experience minimal tissue trauma. Recovery with tumescent liposuction will be faster and post-treatment discomfort will also be reduced.

Recovery

Implant Rupture 

Both saline and silicone implants can technically rupture, develop tears or holes, or suffer other types of damage. Implants are soft by design in order to mimic the natural breast tissue, and they have a fluid core, which may either be saline solution or silicone gel. 

If the saline implant ruptures, the implant deflation will occur quickly and you will come to know about it because the breast appearance will visibly change. Breast implant removal cosmetic surgery can be performed, which will involve a relatively small incision because the implant to be removed would have already deflated. 

In case of silicone implant rupture, you may only come to know about it during your routine MRI exam (which is required every two years with your silicone implants). Removal of silicone implants will involve a slightly larger incision, but your surgeon will first aim to use the old incision lines which were placed at the time of your primary breast augmentation

 

Capsular Contracture  

When your implants are originally placed in the breast tissue, a scar tissue formation will occur naturally around the implant as the body’s healing response. This is known as the breast capsule, and your implant will be encased in this capsule. 

In a few cases, this capsular tissue may inexplicably tend to harden, causing pain or tightness in your breasts. In most cases, a capsular contracture can only be addressed with a breast implant removal or replacement surgery.

The surgeon may remove the entire capsular tissue along with the implant (total capsulectomy), or only some part of the capsular tissue (partial capsulectomy) along with the implant.

In exceptional situations, an en bloc capsulectomy technique will be performed, where the implant and the breast capsule are removed at the same time as a single unit. 

 

Implant Removal with Breast Lift Surgery 

If you have decided not to undergo breast implant replacement, and choose only breast implant removal as an independent procedure, it may sometimes pose aesthetic challenges. 

If your breast tissue has become loose, causing your breasts to sag over the years due to heavy implants, implant removal may result in your breasts looking deflated with the nipples pointing downwards. 

This condition can be corrected by combining your breast implant removal plastic surgery with breast lift. Breast lift will involve excision of your loose skin from the breast along with repositioning or resizing of the areolas (if necessary). If you choose breast implant exchange or replacement surgery, you may be able to avoid this additional breast lift procedure.

How often do you need to replace breast implants?

There is no single accurate answer to this question because individual results after breast augmentation are bound to vary. 

But in general, if you have chosen a skilled and experienced breast surgeon, the results of your breast augmentation plastic surgery are likely to last for anywhere between 10 and 20 years or more. This, however, does not take away from the fact that a few patients will develop implant related complications within their first 10 years, and will require implant removal or replacement.

Will You Need Implant Replacement Every Few Years?

It is a common myth that your breast implants are only likely to last for a few years. The reality is that although you may have to remove or replace your implants at least once in your life, you can expect them to last for decades without any complication. 

In 2011, an FDA study showed that only about one in five women with breast implants will need a breast revision surgery in their first 10 years after the augmentation procedure. This data already shows that at least eight in 10 women can expect their augmentation results to last for more than 10 years, and probably much longer. 

More importantly, you should recognize that implant qualities and surgical techniques have dramatically improved in the last decade (since this FDA study was conducted.) Clearly, the average longevity of breast implants would have further improved during this period, even though there may not be adequate hard data to support this. 

It is also pertinent to note that breast implant manufacturers now commonly offer lifetime warranty on the implants. They will only do that when they are confident of the quality of their implants because otherwise they are the ones who would be placed in a difficult financial position if they have to honor the warranty and provide new implants for free every few years. 

 

Why do Some Women Need to Replace Implants?

Implant complications such as rupture, capsular contracture, shifting, and rippling may not leave you with much choice but to have your breast implants removed or replaced. 

However, you should know that a vast majority of women seek breast implant removal or breast implant replacement cosmetic surgery out of their own choice, and not because they suffered any medical or implant related complication.

Aging, pregnancy, gravity, weight fluctuations, and other factors are going to take their toll on a woman’s breast tissue over time. The en bloc technique was first developed as a method to take out ruptured silicone implants without infecting the body with silicone gel. This procedure involves taking out the capsule (the scar tissue that forms and surrounds the implant) and breast implant combined as one with the aim that no residual tissue is left behind.

The en bloc capsulectomy typically begins with an inframammary incision pattern, which is formed horizontally beneath the natural fold of the breast. If the patient previously has an inframammary pattern from her original breast augmentation surgery, the surgeon will try to make use of the same incision site to minimize new scarring. The surgery usually takes less than two hours and is performed with the patient under general anesthesia.
